Rhinoplasty, or a nose job, is a procedure used to correct deformities of the nose. Its main purpose is to improve the appearance of the nose and, in some instances, to improve nasal breathing. Primary Rhinoplasty refers to first-time nasal surgery, whether for cosmetic or medical purposes. Secondary Rhinoplasty is for individuals who are dissatisfied with the results of a previous rhinoplasty procedure.
Over the years Rhinoplasty has been subject to trends, just as in fashion. In the recent past, scores of women thought it desirable to have an upturned nose. Later, the trend was toward a natural, strong nose with a high bridge and sculpted tip. Today, we know that the perfect nose is different for each person. Primary rhinoplasty is accomplished by removing excess bone and/or cartilage or, in some cases, adding cartilage grafts to help remodel the nasal framework. After alteration of the supporting structures, the skin is allowed to redrape over the newly shaped framework to give the nose its new appearance. The changes may be subtle or dramatic, depending on the needs of the rhinoplasty patient.
The doctor's goal is to produce a nose that appears natural, functions properly, and is in balance and harmony with the other facial features. The "operated look" is avoided. The nose job patient's wishes are always considered, but there are limiting factors. This procedure is performed to correct problems that persist or develop after a previous rhinoplasty. Sometimes the problems are minor and easily corrected, but if they are major, the secondary rhinoplasty is always more difficult and requires more expertise than the primary surgery. This is due to the fact that the normal anatomy is no longer intact after the primary nose job and the tissue planes present before the first surgery are now filled with scar tissue. This makes it difficult to separate and reposition the remaining cartilage and bones. Using the proper techniques, significant improvement and, sometimes, dramatic results can be achieved after the second nose job surgery.
Since most secondary rhinoplasty patients have an operated and unnatural looking nose that does not function properly, Dr. Gunter's surgical goal is to produce a nose that appears natural, functions properly, and is in balance and harmony with other facial features. To accomplish this, it is necessary to reconstruct and reshape the supporting framework of cartilage and bone and allow the overlying skin to redrape over the reconstructed framework to give the nose its new shape. The patient's wishes in regard to the type of nose desired are always considered, but there are limiting factors such as skin texture and thickness, the amount of scar tissue from the previous surgery, and the facial proportions and contour.
