1. Get the 6 necessary photos taken. It is imperative you use a portrait lens (105mm) with adequate lighting and a solid color background or a professional photographer for these shots. A normal point and shoot (35mm) lens will distort the face and nose too much for an accurate evaluation. Any portrait studio (Olin Mills, K-Mart, etc.) should be able to provide these shots easily and at a low cost. Print photographs are required.

2. The photos needed are: 2 Lateral (side view), 2 oblique (3/4 view), 1 frontal and one basal (showing the bottom of the nose). Make sure the pictures are head shots and not full body. See following example:
